A Case Study That Changed Everything

When Traditional Psychiatry Meets Integrative/Functional and Metabolic Medicine

In 2018, I was practising as a psychiatrist in Jaipur and had just started learning integrative/Functional and metabolic Medicine from A4M.

I am sharing a longitudinal case study of one extraordinary journey from 2018 that offers a glimpse into the potential of integrative and metabolic approaches to mental health care.
The Challenge: When Nothing Seemed to Work
Meet “RN” (name changed for privacy) – a young man whose battle with schizophrenia had lasted 4-5 years. Despite multiple hospitalisations and various medication trials by experienced psychiatrists in Jaipur, his symptoms persisted.

His devoted sisters brought him to our clinic, their faces etched with years of worry and diminishing hope.

The conventional approach had reached its limits.

Multiple antipsychotic medications, dosage adjustments, and standard psychiatric interventions yielded minimal improvement. I even tried changing medicines for a few months, but nothing was working. It was time to think differently.
A Functional / Metabolic Medicine Doctor’s Perspective
It is where functional medicine psychiatry entered the picture.

Rather than simply managing symptoms, we began investigating the root causes behind his psychiatric presentation.

Our hypothesis?
Many psychiatric disorders may involve mitochondrial dysfunction triggered by:
Chronic infections
Environmental toxin exposure
Intestinal hyperpermeability (“leaky gut”)
Genetic variations (SNPs)
Hormonal imbalances
The Investigation That Changed Everything
When the Family insisted on comprehensive testing, we conducted:
Routine blood work
Heavy metal toxicity screening

Elevated HsCRP (indicating inflammation) and significantly raised serum aluminium levels.
The integrative/ Functional Medicine Protocol
Instead of adding more psychiatric medications, we focused on addressing the underlying biochemical imbalances:

Phase 2 Detox Support:
N-acetylcysteine (powerful antioxidant)
Alpha lipoic acid (mitochondrial support)
Phosphatidylcholine (cellular membrane repair)
Magnesium supplementation
Vitamin D3 optimization
Probiotic
Gluten-free casein-free diet
Along with conventional psychiatry treatment
The Remarkable Transformation
Timeline of Recovery:
Months 3-6: Gradual symptom reduction
Year 3: Significant improvement in cognitive function
Years 4-8: Progressive return to functionality

Today, eight years later, this young man is not just symptom-free – he’s starting his own tiffin centre business in Jaipur.
Why This Matters for Families and Healthcare Providers
These outcomes remind us why we chose medicine. When former patients return for follow-ups with genuine smiles and stories of rebuilt lives, it reinforces our commitment to exploring all therapeutic avenues.

For families seeking hope: This case demonstrates that treatment-resistant doesn’t mean treatment-impossible.

For fellow healthcare providers: It highlights the importance of investigating underlying biochemical factors in psychiatric presentations.
